In November, we applied for a CLIA waiver that will allow Escuela staff to test students and staff for both Test-to-Stay and surveillance testing. At the end of January, our waiver was granted, and we have received supplies from Premier Medical Group so that we can begin in-house testing. This is wonderful news, as Escuela staff will now be able to provide more consistent testing.
- Because PMG will no longer be administering testing, to participate in Test-to-Stay and surveillance testing, you will need to register your child again using Simple Report. We are unable to use the PMG permission forms for our in-house testing, as we must report directly to the CDC.
- Please note: you do not need to know your student’s ID number to register.
- We ask that you fill out this form as soon as possible so that your child can continue to attend school when a Test-to-Stay protocol is enacted.
- If you only want your student to participate in Test-to-Stay and not surveillance testing, please email the Office.
