Medication Form Request

Thank you for your interest in submitting a medication form. Your child’s safety is our highest priority, and we appreciate your support in keeping both the office and teachers informed of your student’s medical needs. If your child is receiving medication for a limited amount of time, please follow the short-term medication form link below. If your child requires long-term/permanent administration of medicine, please contact the office to send in your authorization as well as a physician’s note.
